Church of the Transfiguration is proud to offer CYO boys’ basketball teams for elementary and middle school students.

Registration:  In the Parish Hall on Thursday, September 28th from 6:30 to 8:00PM

Practice: Begins the 1^stweek of October.

League Play: November 2017 through the end of February 2018.

Playoffs: tbd

Registration Requirements

1. Copy of a birth certificate (first year participants)

2. Copy of Parish Boundary Map with your home marked on the map

AND

Copy of a utility bill for proof of residency

OR

Enrollment in CCD at Transfiguration

3. Transfiguration CYO Registration Card

4. Diocese of Oakland Basketball Release Form

5. Signed CYO Code of Conduct Form

6. Signed CYO Concussion Information Form
