Registration Requirements

1. Copy of a birth certificate (first year participants)

2. Copy of parish boundary map with your home marked on the map

AND

Copy of a utility bill for proof of residency

OR

Enrollment in CCD at Transfiguration

3. Transfiguration Registration Online

4. Diocese of Oakland Track Release Form

5. Signed CYO Code of Conduct Form

6. Signed CYO Concussion Information Form

Parent participation is necessary as track meets are all volunteer sponsored.
